  • The Initiative Nominee (I.N.) for Safety award was created to recognize UCLA faculty, staff, student or volunteers who demonstrate outstanding dedication to laboratory and safety awareness.
  • How do I nominate someone?
  • Complete this Nomination Form.
  • Who decides the winner?
  • The Office of the Vice Chancellor for Research Safety Oversight Committee (OSOC) will select the final award winner(s).
  • What is the prize?
  • A small monetary reward and invitation to join fellow Safety Forward participants at a luncheon hosted by EH&S
  • When will a winner be announced?
  • Winners will be announced quarterly via email and also in the Safety Forward Winners page.
  • Can I nominate myself?
  • Yes, we want to acknowledge your efforts in safety. Let us know what you have accomplished
