UPDATED: Changes to the Medical History Questionnaire (MHQ)

The MHQ is a requirement for all personnel involved in animal research activities including animal care, access to vivarium spaces, handling animal tissues, sponsors of animal research and those that support animal research. The MHQ is reviewed by Occupational Health Services (OHS) and the Arthur Ashe Student Health & Wellness Center (Ashe). These medical professionals will ensure proper medical surveillance based on risk and medical history.

Updates taking effect August 20, 2024

  • If you work exclusively with “all other animals such as purpose-bred rodents, rabbits, dogs, cats, reptiles, and fish etc.” they are considered lower-risk research animals, and you are now required to fill out a new MHQ every three (3) years.
  • All others are required to fill out a new MHQ annually.

Any approved submissions after August 20, 2024 will see the updates reflected on their MHQ. Any approval prior to this date will not see the changes and will have an annual expiration regardless of the category selected.  

Fill out a new MHQ if you work with a new species, have significant changes in your health, or worsening of an ongoing medical condition. You are required and fully responsible to update your health information and/or risk category regardless of expiration date.
